When miles or circumstances prevent you
from being with your loved ones, our team of professionals will manage and
oversee all aspects of care for your elder. We provide experience

ElderLink is a unique,
personalized service, offering adult children valuable solutions to the
multiple care needs of their elders. Trained professionals, with
experience, dedication, and patience, will help- when miles or
circumstances prevent you from being there.
Assessment
In the initial steps, experienced Care Mangers meet with the elderly
person and their immediate family members, gathering all pertinent
information needed to advise the elder and their family members.
Review
The assessment reviews many facets of the older person's situation, such
as life-style, wishes, finances, mental health, physical prognosis,
medication, physical history, family support and systems, etc.
Action Plan
After assessing the situation, we draw upon our vast knowledge of Home
Care, LTC Insurances, Medicare, Medicaid, Nursing Homes, and Assisted
Living facilities to offer a plan of action (or care plan) for the life of
the elder. This plan will assist the family and elder in selecting the
right options.

Geriatric Care Management
Following an assessment, families may want to contract with ElderLink to
support or manage the elder's care needs.
Ongoing Services
We offer ongoing advice and support to the elder and their family. Whether
the elder is living in a facility or in their own home, we can spot-check,
hire, and manage household staff.
ElderLink can, if appropriate, organize and
implement a healthy balanced-care plan that everyone can live with.
Continuing
communications
Often, we communicate with the family (with the permission of all
involved) on a regular basis. Whether the family lives locally or at a
distance, ElderLink involvement can allow you to remain “family” - while
we take care of the clinical information.

Guardianship
ElderLink offers ongoing, professional guardianship services.
Organized through a court appointment,
guardianship is an oversight arrangement. This arrangement may be
voluntary (the elder requesting a guardian due to physical frailty), or
Involuntary (due to an elder’s incapacity).
Either way, your loved one is assured of
the care and protection professional, court-approved guardianship affords.
